Spectrum Generations’s Bridges program works to suit your short or long-term care needs when home is where you want to be. Bridges provides personal services to adults wishing to stay and/or recover in their own homes. We can provide as little as two hours a week or 24-hour, round the clock assistance. Our bonded, trained, certified, and supervised Personal Service Specialists (PSS’s) are trained to deliver:
- Homemaking
- Meal preparation
- Laundry
- Shopping/errand services
- Companionship
- Gentle guidance/medication reminder/cueing
- Transportation to appointments
- Assistance with bathing/dressing
- Overnight care
Care Management
Bridges Care Management can provide an individualized, in-depth study of issues, concerns and needs that pertain to you or a family member. We offer quality support from staff trained in the medical and social service fields. Our Care Managers focus on the client as a unique individual and will assist in developing a specific plan of care. We work closely with family members, health care providers, trustees, guardians and a carefully screened network of community services.
Personal Emergency Response System
Bridges also offers the Personal Emergency Response System (PERS). This convenient device can be worn around the neck or the wrist and provides an immediate response for emergency crisis situations. It’s a cost-effective and reassuring solution for those who live alone.
Care Managers can assist older individuals and their caregivers with the following:
- Have multiple needs or already have services that need monitoring.
- Contemplate a change of residence or placement in a care facility.
- Need an adequate and dependable informal support system.
- Be overwhelmed by complex family dynamics or living far away from family.
- Live alone.
- Be home bound due to physical or mental limitations.
- Exhibit poor judgment.
- Be unable to access care.
- Require personal advocacy and support.
